Report Benefits Fraud

Use this form to report suspected public assistance or benefits fraud. Public Assistance fraud occurs when a provider or individual knowingly makes false or misleading statements to obtain payment or benefits from a public assistance or benefits program. Examples include billing for services not provided, charging more than the reasonable value of services, or providing unnecessary services.

Please provide as much detail as possible, including:

  • Who is involved
  • What public benefit or program may be affected
  • What you personally observed
  • When and where it occurred
  • How you became aware of the situation
  • Supporting documentation to the claim 

Your Information

If you provide your contact information but wish to remain anonymous, the Auditor of State's Office will keep your identity confidential, along with any information that could reasonably reveal it under ORC § 149.43(A)(2). Providing contact information is not required, but we encourage you to do so, as the scope of our inquiry may be limited without the ability to follow up for additional details.
